ID and Age Requirements

Age Requirements
For those 18 years of age or older, we require a valid state—and government-issued photo ID. Your ID will be needed every time services are rendered.
If you are under 18, please review the documentation requirements for piercing minors. Failing to provide the necessary documents will result in the cancellation of your appointment and the forfeiture of your prepayment. Please refer to the examples below or contact the studio with any questions.
Identification Requirements
If you are 18+ older, you must have one of the following valid (not expired) forms of identification:
1) Driver’s License
2) State Identification Card
3) Passport
4) Greencard
Minors — Parents, we kindly ask you to read through all the information below related to piercing minors!
If you are accompanying a minor, the State of California requires that you have the following accepted forms of ID and documentation requirements:
Minors Ages 8-13 (Earlobe Piercings Only)
Parent/Guardian's valid, government-issued photo ID (hard copy).
Minor's original Birth Certificate (hard copy or electronic accepted).
Guardianship Exception: If the adult present is the minor's court-appointed Legal Guardian (not a parent listed on the birth certificate), original court-ordered guardianship paperwork must be provided instead of the birth certificate to establish the right to consent. All other ID requirements remain.
Minors Ages 14-17 (Most Ear, Nose, Lip, Eyebrow, Navel Piercings)
Parent/Guardian's valid, government-issued photo ID (hard copy).
Guardianship Exception: If the adult present is the minor's court-appointed Legal Guardian (not a parent listed on the birth certificate), original court-ordered guardianship paperwork must be provided to establish the right to consent. All other ID requirements for the minor and guardian still apply.
AND EITHER:
Option 1: Minor's valid, government-issued State ID or Driver's License with matching last name and address to the parent/guardian's ID.
OR Option 2: Minor's photo ID (State ID, Driver's License, Passport, School ID, Yearbook photo page with name) PLUS the Minor's original Birth Certificate (hard copy or electronic accepted) - unless guardianship paperwork is being used per the exception above.
